Include the following information:

1. Model name
2. Serial number
3. Invoice number, date of order, and purchasing company/individual
4. Operating system (MSW XP SP2, Linux Debian kernel 2.6.3, etc.)
5. Description of the problem including error messages
6. Output of the Debug window, if relevant (the output can be saved to a file
   using the File menu)
